	Summary of action required	If Sickness Insurance Scheme has been purchased If your school has returned it’s Contract to Purchase, and has chosen to take out Sickness Insurance cover for 2002 – 2003, the attached claim forms need to be completed and returned on a calendar month basis. If the Contract to Purchase has not been returned The school should return their Contract to Purchase as soon as possible, indicating if Sickness Insurance cover is required.
