Summary of contents

Enclosed is the new Norfolk LEA standard Admission Register Form. This form, in electronic format on the floppy disc, is being made available to all Norfolk LEA schools for use when collecting information from parents about children being newly admitted to your school.

The form has been designed to enable all schools to collect the same core data about pupils and be able to respond to DfEE and LEA data collection requirements. You are free to add more fields to the form, which is in Microsoft Word 97 format, should you need to collect additional information for your own purposes.

Please make this form available to those members of staff who have responsibility for collecting pupil information from parents at the point of admission to the school. It is important that the information collected on this form is entered into your Phoenix Gold Database or other management information system.
