	Summary of action required	Please complete the Norfolk produced post-threshold teacher snapshot form by 30 September, 2001; The nominated governors should carry out a review of the Headteacher’s performance in respect of the academic year ending 31 August 2001 and decide if a performance point should be awarded; claim forms to be completed by 31 December, 2001. The Headteacher should carry out a review of the Deputy(ies) and Advanced Skills Teachers (ASTs); (by 31 December, 2001) Carry out round 2 threshold assessments and complete a claim form by 30 April, 2002; Carry out an individual teacher review, if applicable, and complete a claim form by 30 April, 2002; Supply forms, issued by the LEA on a termly arrears basis, for Headteacher/Chair authorisation; SUPPLEMENTARY2 form to be returned to the LEA on a termly basis.
