I enclose the Summary and Eligible pupil list forms for the Spring Term 2000, which must not be completed until the count date of Thursday, 20 January, 2000. Any forms completed before this date will be returned. Please ensure that completed forms reach us by 2 February, 2000. Failure to meet this deadline may jeopardise payment of your claim.

Eligible children are those born between 1 January, 1995 and 31 December, 1995. Children who are part of phased entry into full time primary education should not be included on this form.
