Annual Review Forms
Further to MI Sheet 123/12 new forms have been created for all Annual Reviews. Please refer to the Annual Review Process Guidance Document for guidance on completing the forms.

Early Years Annual Review Form
A new form has been created for settings and schools to use for Early Years Reviews (to take place at least every 6 months).
This form should be used for all Annual Reviews up to the end of the Reception Year.
Annual Review Form Years 1 to Year 8
This section has been updated to include one form for schools (mainstream and complex needs) to use for Annual Reviews.
Annual Transition Reviews and Plans (Year 9 onwards)
The final version of the Annual Transition Review Form, Learning Difficulty Assessment (LDA), Transition Planning Information Sheet and completed example Transition Plans are now published on the Transition Review and Plans section of the Norfolk School website.
Over the past year some schools have been piloting a combined Transition Annual Review and Learning Difficulty Assessment (LDA). We have also taken the opportunity to amend the Transition Review document which now includes the Transition Plan.
We have received a great deal of helpful feedback on these forms and have reached the end of the pilot stage.
We have decided not to incorporate the LDA within the Annual Transition Review Form. Guidance Advisers will have responsibility for completing a young person's LDA after first seeking out information from other professionals who are involved with the young person.
We have been working in partnership with Adult Services in amending the Transition Review to make sure that the information contained in the review will inform future planning for a young person as they move into adult life.
You will also notice that the new form is significantly shorter than the current one - this is in part due to the deletion of the LDA but also because we are no longer asking for detailed information at the beginning of the review report.
We are instead encouraging schools to take the opportunity the Annual Transition Review offers to confirm the personal and contact information they hold on a young person.
