Transition Planning for young people with additional needs

Changes and/or updates regarding Transition Planning will be sent directly to SENCOs in each mainstream secondary school and headteachers within special schools over the next two weeks.

Two significant changes schools will need to know about include:

Transition Plans – To ensure the Transition Plan is “fit for purpose” we have redesigned it in order to request further information. It is acknowledged that schools have limited time to complete the Transition Plan however we are also aware for the need of vital information to be shared. Both Individual and County Planning rely heavily on the information supplied at reviews and the Transition Plan will be used to inform providers regarding future needs and assist in identifying unmet needs across the county. A copy of the Annual Review (incorporating the Transition Plan) must be sent to county SEN departments in order for this to take place (address of where the Form should be sent can be found on the back of the review paperwork) We are striving to ensure that the planning process is effective as possible for young people and the absence of the review paperwork may seriously inhibit this process We are also informing schools that, from October 1st 2012, any review paperwork that is not on the correct form will be returned to the school.

Transition Notification and Information Form - The Norfolk schools website now has information regarding the Transition Notification process to be used when/if alerting young people to Adult Care. The form is to be used to highlight young people who may be in need of further social care support post 18. This will also be used by Children's Services staff, however, if no involvement is apparent, the school will be encouraged to send the notification themselves.

Every Senco within mainstream secondary schools will also be receiving a pack of new resources and information. The pack will consist of :
  • A Transition ‘Moving On’ pack. The pack provides a broad base of information about the Transition process. This is distributed to the complex needs young people and their families in year 8/9
  • Transition Timeline. This is a resource that has been sent out to all parents/carers of young people with additional needs between the ages of 13 and 19 years of age. The purpose of the timeline is to provide parents/carers with basic key points to consider when planning for the future. It gives a generic overview of specific stages of the process, what should happen and what they should be considering at different points
  • Transition Passport. This resource has been developed for school staff to embed within the curriculum. It is a tool to collate vital information from the young person that can be shared with future support services and providers. Completed on a ‘ as and when basis’ it builds a holistic picture of the young person , information that can easily be transferred into the Transition Plan to avoid duplication.


  • N.B- This resource has recently been adopted by the National Autistic Society and is used in all of their schools and services with the recognition that it is as a very effective, person centred document.
  • Young Persons booklet - Within the Transition Pack you will find a resource that can also be used by schools (and parent/carers) that assists in explaining the changes that may happen during Transition. The booklet explains who may become involved and why. It has also been used to explain to parents/carers about Transition who may also have learning disabilities themselves.
  • Multi - Agency Protocols - the Norfolk Protocols form the governance over the Transition process and provide a holistic view of the roles and responsibilities of key agencies. School staff should be made aware of the protocols as it provides vital information on who should or may be involved and what should be happening at specific stages of the young persons life for effective planning to take place.
