Management Information Sheet

In light of feedback from some schools about the annual review forms for children with statements of SEN, a number of changes have been made.

These include:

• The contact details page has been reinstated
• The chronological age group field reinstated
• The date of phase transfer and date of last Annual Review has been added
• A draft or final drop down box has been added to page 1
• Pupil name entry field added to page 1
• The role column for attendees has been left blank to be completed by the person filling the form in
• Additional rows added to the targets column and action plan
• Heading changed to state date or term on the targets coloumn

Please note that all forms, processes and procedures will be reviewed as part of the Children and Families Bill. In the interim schools should continue to use the forms available on the Norfolk Schools website.
