Norfolk Family Focus referrals
Norfolk Family Focus has revised the process for making referrals into the service, in order to make it more straightforward for both families and professionals. When school staff identify a family which meets the criteria for inclusion on the programme, they will need to fill out the attached Request for Support (RFS) form with the family and talk them through the attached Privacy Notice, which details what uses their personal data will be put to.
It is important that families understand the content of the Notice and some will need more support than others to be able to do this. All, however, will need to be talked through the content - it is not sufficient to just give them the form to read - and will need to sign on the RFS form to say they have read and understood the Notice. We will need signatures of all members of the family who are over the age of 12.
The main criteria for inclusion on the programme hasn't changed (please see the RFS form for full details). However, there are two new additions to the Local Criteria, which are as follows:
- The family meets threshold levels 2,3, or 4.
- The family is in need of an assessment and multi-disciplinary intervention.
