New Education Health Care Plan (EHCP) annual review forms for collecting views from children and young people and parents/carers are now available online. Both forms can be found in the "Annual Review of an EHCP" section at the bottom of the page.

One form is to enable the views of the child or young person to be collected and the other is to enable the views of the parents / carers to be collected.
