Revision to Criteria for Statutory Assessment and Statements of Special Educational Need.

The attached document sets out updated guidance for making requests for statutory assessment and replaces the guidance issued by Norfolk Education Authority in 1998.

The Criteria should be read in conjunction with the Special Educational Needs Code of Practice issued in November 2001, with particular reference to Chapters 7 and 8.

The document takes effect from 1st November 2005. Schools and settings should begin all new referrals for statutory assessment using the new forms. The previous 'Stage 4' forms will no longer be accepted for referrals after 31st December 2005.
