Referral Forms for Statutory Assessment

Norfolk published its SEN Criteria in November 2005. The Criteria document includes new referral forms for Early Years and School Age Pupils. All referrals for statutory assessment must be made on these forms. Some schools and settings are still using old 'Stage 4' forms which are out of date.

Referrals made on such forms will not be accepted after 31st October 2006 and will be returned to the referrer.

The new forms can be downloaded from Esinet. Ideally forms should be completed electronically and then printed off for signature. It is not possible at present to accept referrals via email. Schools and settings are reminded to read the SEN Criteria document carefully before submitting a referral to ensure that there is clear evidence that the criteria have been met.

It is also important to ensure that all sections of the referral form have been completed and that the form has been signed. Unsigned forms will be returned to the referrer. If photocopies of supporting documents are submitted as evidence they must be legible throughout.

The checklist at the back of the referral form must also be completed.
