Transition Plans and Reviews for all Pupils with Statements of SEN in the Current Year 10

All pupils with statements of SEN must have a Transition Plan completed as part of their Transition Review (Annual Review) 

The SEN Code of Practice 9:4 states that ‘the Head Teacher must ensure that a Transition Plan is drawn up.’ 

Guidance on Annual Reviews can be found in Chapter 9 of the SEN Code of Practice (SENCOP) and Section 10 of the SEN Toolkit. Specific guidance regarding Transition Reviews and Transition Plans can be found from section 9:45 of SENCOP). 

We will be running an audit of our records in the next few weeks to ensure that all pupils who are currently in Year 10 had a Transition Review and a Transition Plan completed during Year 9.  Schools will be notified where we have no record of a Transition Review and Transition Plan and asked to ensure that this has been done by the end of this term.
