As part of the SEND Transformation Project we have reviewed the referral paperwork and have liaised with SENCos and Head Teachers to simplify the forms and make them easier to complete. We are aware that you are being asked to fill in these forms even if you feel you can meet the child's needs and do not think an Education, Health and Care Plan (EHCP) is necessary, therefore we have created a section for you to tell us exactly that without having to fill in lots of details. We have also combined the referral form with all other forms we need to have completed. The referral form will also have the Data Protection Consent form (DP1) and the Family Conversation form attached, so everything is in one document.

In collaboration with the Early Years Team and Early Years SENCos, we have designed an 'Early Years' referral form that is more meaningful for this cohort and should make it a simpler process.

These new forms will replace the current ones you currently use on Norfolk's Local offer and these are now live.

What we are requesting education providers do:

Start using the new forms. We will accept the old format of the forms for four weeks after the new forms are live, but after that we will return them to you as a rejection, so it is important the new forms are used as soon as possible.

Ensure that parents/guardians or the young person if aged 16 or over sign the DP1. We cannot accept the DP1 signed by a member of the school.

Send all the relevant reports you have collated and have quoted in the referral form to:

What the LA will do:

We will keep a record of which schools have completed the referral forms for a period of six weeks and then we will ask you for feedback on the new forms. Feedback will be shared at the SEND Management meetings and we will review the forms and make relevant changes if needed.
