Learning from recently published Norfolk Serious Case Reviews / Safeguarding Practice Reviews

Webinars for Designated Safeguarding Leads

These 90-minute webinars are for trained DSLs. They do not replace DSL refresher training.

In August and September 2020, Norfolk Safeguarding Children Partnership published three serious case reviews (Cases AE, AG and AH) and one safeguarding practice review (Case AI). This webinar will give a brief overview of the reviews, provide DSLs with an opportunity to discuss key themes and reflect on implications for school practice.

It is crucial that DSLs have read all four publications prior to attending the webinar.

  • Child AH died in an accident involving stair gates following a cardiac arrest. The family was a sibling group of five and their parents lived separately but their relationship and additional needs were not well understood. Neglect was a feature of the case; however, the children have never been the subject of child in need or child protection plans.

  • Child AE was sixteen years old who died shortly after taking a paracetamol overdose. Child AE had complex emotional and physical needs and there were multi-agency responses over many years with multiple services, professionals and clinicians involved.

  • Child AI was five years old; she sustained significant burns to 26% of her body. She had previously been subject to a child protection plan under the category of neglect with subsequent child in need plan. Ambulance services and police made safeguarding referrals and the early years setting had recorded 19 cause for concern forms prior to her transition to primary school. There were concerns regarding anti-social behaviour and possible child sexual exploitation.

  • Child AG presented at hospital as severely malnourished and neglected when aged two and half. Child AG is the fourth child in a sibling group of six. The case had been managed under a child protection plan but had been stepped down to a child in need plan. There had been previous concerns about the family including neglect and domestic abuse.

This webinar will be led by Lucy Canning and Claire Farrelly, Safeguarding Advisers from the Education Safeguarding Team.
