Online incident reporting (OSHENS)

This article provides you with information to ensure that incidents are reported correctly. Please communicate this information with your staff; this will help the HSW team to review and support incident investigations and to ensure that the organisation responds to accurate information.

Reporting involuntary harm

It's really important to select the correct type of incident category within OSHENS, this means that we can gather accurate information about incidents. If you need to report a violent incident, please always consider if the incident relates to involuntary harm. This would apply where the individual does not have the capacity to intend to cause the harm that occurred. If you believe that the incident is related to involuntary harm, please select this as the incident type.

Protected characteristics

When you report an incident, you will need to fill section called "what happened". Part of this section asks you to identify if the incident relates to the protected characteristics of the injured/affected party. This section is for employees who are injured/affected only and not the person who may have caused the harm.
