Information re the development of Norfolk Healthy Child Services for 5-19 Years
As part of the development of the Norfolk Healthy Child Programme we have agreed with our Public Health Commissioners that in January 2017 we will commence a new service redesign project 'Re-imagining HCP services for 5-19 age groups'. The re-design will focus upon improving outcomes for those individuals and groups identified at highest risk of health inequality, concentrating upon identified key priorities for the County: emotional health and wellbeing, sexual health and improved health literacy and on the key points of transition; transition to school, transition to high school and transition to adulthood. It is important to us that stakeholders, and of course service users, are involved with our redesign and we will seek your views on the best ways to achieve this.
As part of the service development we will be implementing some immediate changes which we believe will facilitate long term improvements and I have detailed some of these for you. Over the next few months we will be transitioning all our teams to a 0-19 Locality based structure and as part of this work we will provide Named Specialist Community Public Health School Nurses for each of the six localities, to replace the named school nurse per school cluster model. We will provide individuals' names to you as soon as we have completed this work.
There will be no change to the referral process for individual work with young people, please continue to refer through the Single Point of Access (SPOR) using the established processes. All safeguarding support will of course continue to be prioritised.
ChatHealth will continue to provide an important point of direct contact and access for young people and we will increase opportunities to promote this through themed drop ins and distribution of information through our Schools Immunisation Team and our iCASH service. Themed drop ins currently scheduled with schools for this term will continue, but we will discontinue all other drop ins with immediate effect.
Medicines Management training in schools that is already booked for the period January 2017 to March 2017 will continue however as part of the service redesign we are exploring opportunities for signposting to on-line modules. The school nursing team will no-longer provide PSHE support to schools, but we will signpost to resources which have been approved as best practice by the PSHE Association.
Over the next few months we will be arranging Stakeholder Events across the County, and would welcome input and ideas from all those involved with children and young people, as well as young people themselves. These workshops will give us an opportunity to share our ideas but more importantly will offer the opportunity for you to shape our services to best meet the needs of children and young people, and also to ensure that we achieve our goal of improving outcomes for all children and young people.
