East Coast Community Healthcare CIC - Childrens Speech and Language Therapy Referral Process

To ensure we have appropriate written consent and information that will facilitate the telephone triage process, we have introduced a written referral information form, which must be submitted by professionals before they have a telephone triage appointment - please see attached Referral Process for Professionals.

From the 1st January 2018 we will require this form to be completed and submitted, if not received, we will be unable to go ahead with your telephone triage appointment as we require written parental consent.

It is also important that a written referral information form is submitted as this will help guide the conversation, without which, we are unable to discuss the child's needs with you. The attached form can also be accessed on our website Children's SaLT, from 1st January 2018, or it will be sent to you at the time you book a telephone appointment.

Parents can still refer direct as detailed in the attached Referral Process for Parents.
