Post-16 transition support for young people with additional needs
This MI Sheet is being issued as a reminder to schools to use these forms as detailed below.
Please see the attached Transition Support Requirements form and guidance notes. This form has been designed by the new Education Inclusion service for schools to use. Its purpose is to enable the sharing of information with post-16 providers such as Colleges and Training Providers, so that the support requirements for young people in their leaving year with significant additional needs but who do not qualify for an Education Health Care Plan (ECHP) can be appropriately met. Previously this group of young people would have been subject to a Learning Disability Assessment but under new legislation these have now been phased out nationally.
The form should be completed by the school as fully as possible as this will allow the post-16 provider to put in place appropriate support arrangements for the young person based on a thorough understanding of the young persons needs, interests and future plans. It is to be used with immediate effect.
The school should share this in a secure way with providers and it is recommended that this is before the young person starts in provision (ideally at the point of application), in order to give time for support arrangements to be put in place. If a young person is considering a number of options the form should be shared with all relevant providers.
