Development of the Norfolk Healthy Child Programme (HCP)

I am writing to update you on the development of the Norfolk Healthy Child Programme (HCP) model for the 5-19 year old element of our service developed in conjunction with HCP staff, stakeholders, children and young people (CYP) and agreed by our Public Health commissioners.

The new model will be launched in September 2017, and is based upon Norfolk County Council Public Health's objectives to improve outcomes for children and young people, including:

  • Targeting resources to areas and school clusters identified as being in greatest need; alignment of resources to Safer Schools Partnerships priority schools and clusters, complex needs schools, short stay schools and Looked After Children
  • Identified priorities for children and young people in Norfolk
  • Increasing use of technology, such as the texting service ChatHealth to enable all young people to access the service and to promotes greater health literacy
  • Transition to school, high school and adulthood, with services aligned to health reviews at those life stages
  • Signposting schools to online resources and specialist organisations for PSHE, medicines management and training
  • Evaluating the impact of all interventions utilising Goals Based outcomes tools for all interventions with children and young people
  • Developing a workforce which includes greater skill mix
  • Increased partnership work, with service development and resource for complex needs and short stay schools
  • Development of a service for 16-19 year olds

All localities will have a lead locality Specialist Community Public Health Nurse or Nurses for the 5-19 service as well as a skill mix team who will provide individual interventions with children and young people. This makes greater use of our Specialist Community Public Health School Nurses who work within our 0-19 years integrated locality teams and will utilise their leadership skills and strategic public health overview to support understanding of health priorities, partnerships and service developments within their localities.

We are working with our staff to operationalise the new service through recruitment and training, and as part of this work have developed clear referral criteria for individual interventions and care pathways which are attached with this letter.

Referrals will only be accepted in line with the criteria and through Just One Number 0300 300 0123 or email on ccs.nhsp-norfolkcyphealthservices@nhs.net and for 11-19 year olds directly and independently through the ChatHealth service by text on 07840 635060. I have also attached a flyer giving brief details of the locality model for you to share with your staff.
