Urgent - Information Required Re: School Leavers Post 16 Transport
Dear SENCO/TITAN Leader/Careers Advisor,
I am writing to request urgent information in regard to the post 16 transport of your 2015/16 school leavers.
We are now past the deadline that we require full referrals for all students needing any support with post 16 education. Providing the referrals has been the responsibly of our team of EHCP coordinators. Unfortunately due to other critical work pressures they have, in the most part, been unable provide these.
Due to this we are now asking that, where possible, schools support us by completing the referrals. It is important we receive these referrals urgently, by Friday 13th May. Any referrals after this date will still be accepted, but are not guaranteed TITAN support and may delay the start of their transport in September.
You may have already provided us with summary information in regard to students who require some support, for us to action this we now require the full referral.
A referral is required for students who may need bespoke transport or TITAN Buddy support. If you are unsure as to the needs of the student, please refer them to TITAN. The TITAN Buddy Service will support them on a one to one basis throughout the summer in developing their confidence and independence. If by the end of the summer they are not a fully independent traveller, eligible students will be provided with bespoke transport. This is a free service available to all students entering into 2016/17 post 16 education from September who may need transport support. A two leaflets for your information are attached.
Once the referrals have been received, we will write to parents/guardians to provide information on the service. We will then visit their home, to make introductions and discuss any concerns. Once referred, families can opt out of the service at any time.
